fill-in-the-blank document to help you organize your goal-setting thoughts and
goal development. You'll write on it your final goal statement and your
action plan to meet that goal. Make copies of the form and use one form per goal.
You'll need to complete all but the last line of this entire form before
you start working on achieving your goal. Once completed, it'll help keep
you on track for achieving your goal.
